Service enquiries

Contacting PADL does not establish a clinical relationship or confirm a service booking. Suitability, service area, availability, scope and fees are discussed and agreed separately before services begin. Any service agreement and consent requirements are provided as part of that process.

Funding and service arrangements

PADL service coordination is separate from care management. Your registered Support at Home provider retains responsibility for approvals and oversight. An enquiry does not confirm funding, service availability or payment arrangements.

External services and availability

The enquiry form is hosted by Microsoft. Links to other services are provided for convenience and those services have their own terms and privacy notices. If a page or form is unavailable, use the contact details on our Contact page. This website is not an emergency or urgent care service.
